What: ATHLETIC PHYSICALS

When: May 21st from 5PM - 7PM

Where: Wekiva High School in the Gymnasium (Building 3)

The cost is $15.00- If you are paying by check, please make checks out to Wekiva High School.

We will start giving numbers out at 5 p.m. We will be using the first come, first serve process, so be sure to get their early. To get a physical you MUST be in line no later than 7pm!

Physical packets can be picked up in the front office or room 315 in the gym hallway. They are also available online at www.wekivamustangs.com in the forms tab in the Sports Medicine section. If the paperwork is to be filled out on-site, students must have a parent present.

This will be the only school physical given by Wekiva High for the year, so make sure you don't miss it!
